- Clonazepam may be administered concomitantly with one or more antiepileptic agents. However, adding an extra drug to a patient’s regimen should involve careful evaluation of the response to treatment, since unwanted effects, such as sedation and apathy, are more likely to occur. In such cases, the dose of each drug should be adjusted to achieve the desired optimal effect. Pharmacokinetic interactions: The antiepileptic drugs phenytoin, phenobarbital, carbamazepine and valproate may increase the clearance of clonazepam thus causing lower plasma concentrations of clonazepam during combination treatment. Clonazepam per se is not an inducer of the enzymes involved in its own metabolism. The selective serotonin reuptake inhibitors sertraline and fluoxetine do not affect the pharmacokinetics of clonazepam when administered concomitantly. Pharmacodynamic interactions: The combination of clonazepam with valproic acid may occasionally cause petit mal and status epilepticus. The sedation, respiratory and hemodynamic effects may be accentuated when clonazepam is co-administered with centrally acting depressants, including alcohol. Alcohol should be avoided in patients receiving Clonazepam (see General Precautions). In combination therapy with centrally acting drugs, the dose of each drug should be adjusted to obtain the optimal effect.

Simple and social phobia, specific phobia. Generalized anxiety disorder. Insomnia. Clonazepam is indicated for the treatment of epilepsy as a first-line medication in typical absences (formerly called petit mal), atypical absences (Lennox-Gastaut syndrome), myoclonic seizures, and atonic seizures (drop syndrome), as a second-line agent in infantile spasms (West syndrome), and as a third-line agent in tonic-clonic (formerly called grand mal) seizures, simple and complex partial seizures, and secondary generalized tonic-clonic seizures.
